To help seniors stay in their homes for as long as possible by:

• Long-range planning to minimize crises from occurring

• Planning financially, so that the client's resources last if possible for care needs

• Advocating for clients and their families to allow them to achieve the goals they have set for themselves with the assistance of a care manager

• Using an interdisciplinary approach that addresses all the client's needs as fully as possible

• Aiding in creating, implementing, and monitoring a Plan of Care (POC) to ensure it continually meets the needs and desires of the client

• Aiding with choosing and monitoring of alternative arrangements when remaining in the home is no longer practical or appropriate 

• Confidential needs assessment and coordination of specialized services

• Planning, education, and advocacy

• Facilitate & provide family caregiver coaching for complex family dynamics 

• Coordinate, evaluate, and monitor living arrangements

• Promote caregiver stress relief by coordinating respite 

• Coordinate transportation to medical appointments

• Able to facilitate groups/educational workshops

• Gaudian Ad Litem Assessments and Court Appointed Guardian
